At Bellarine Active Ageing OT, we support older adults in maintaining safety, independence, and confidence within their own homes. Our mobile occupational therapy services are specifically designed for individuals on the following programs across the Bellarine Peninsula

Meet Jen
Jen is a dedicated Occupational Therapist who graduated from Charles Sturt University in 2018. With a strong passion for aged care, she is committed to empowering older adults to live independently and achieve meaningful goals in their daily lives.
Jen’s experience spans a variety of government-funded aged care programs, including Short-Term Restorative Care (STRC), Commonwealth Home Support Programme (CHSP), and Home Care Packages (HCPs). Jen brings a holistic and client-centred approach to every aspect of her work.
Before becoming an OT, Jen built a diverse career in health and human services. She has worked as a professional counsellor, workplace trainer and assessor, and a registered nurse (Division 2). Her background also includes case management and support worker coordination, giving her a well-rounded understanding of the aged care landscape.
Outside of work, Jen enjoys spending time in nature—whether it’s walking along one of the local beaches, hiking through scenic trails, camping under the stars, or flowing through a Pilates class at her local studio.

Collection of personal information
• Bellarine Active Ageing OT may at times, receive and store personal information you enter into the ‘Client Referral’ and ‘Contact Us’ pages on our website, provided to us directly or given to us in other forms
• The ‘Client Referral’ page on the website requires detailed information to enable Bellarine Active Ageing OT to provide relevant OT services to the nominated client
• The ‘Contact Us’ page on the website requires you to provide basic information such as your name, phone number and email address to enable us to contact you for further information or book an appointment
• Importantly, we will be collecting sensitive personal information relevant to client care and treatment at in-person and/or Telehealth appointments essential for the work we undertake
• We may also collect any other information you or your caregivers provide while interacting with us via email and/or phone calls
• Bellarine Active Ageing OT recognise the Australia Privacy Principles that recognise individuals have the option of not identifying themselves, or of using a pseudonym, however, services are not able to be provided in this instance
How we collect your personal information
• Bellarine Active Ageing OT collects personal information from you in a variety of ways, including when you interact with us electronically via email or in person, when you access our website and when we provide our services to you
• We may receive personal information from third parties, such as referrals from case managers and support coordinators. In these instances, we will protect it as set out in this Privacy Policy
Use of your personal information
• Bellarine Active Ageing OT may use personal information collected from you to provide you with information and our services
• Bellarine Active Ageing OT will not send you advertising material based on services, updates or irrelevant information to your care
• Bellarine Active Ageing OT may contact you by information that has been provided to us; be that by telephone, email and/or SMS
• Bellarine Active Ageing OT will keep your personal information as long as we are legally obliged to keep it, or until the need for the personal information has passed. At that point it will be destroyed, deleted or de-identified
How we disclose your personal information
• We may disclose your personal information to our insurers and/or professional advisers, insofar as reasonably necessary for the purposes set out in this Policy
• Personal information is only supplied to a third party when it is required for the delivery of our services (i.e requesting quotes for equipment/home modifications, referral to other allied health professionals, requesting joint builder visits, contact with treating GP when indicated or mandatory reporting etc.) or in other rare instances such as the need to employ a debt collection service
• Bellarine Active Ageing OT will include necessary personal information on formal documents such as reports, and share this with the intended audience (I.e. HCP providers, NDIS, funding organisations)
• We may from time to time need to disclose personal information to comply with a legal requirement, such as a law, regulation, court order, subpoena, warrant, in the course of a legal proceeding or in response to a law enforcement agency request
• If there is a change of control in our business or a sale or transfer of business assets, we reserve the right to transfer to the extent permissible at law our user databases, together with any personal information and non-personal information contained in those databases. This information may be disclosed to a potential purchaser under an agreement to maintain confidentiality. We would seek to only disclose information in good faith and where required by any of the above circumstances
• By providing us with personal information, you consent to the terms of this Privacy Policy and the types of disclosure covered by this Policy. Where we disclose your personal information to third parties, we will request that the third party follow this Policy regarding handling your personal information
• We do not and will not sell or deal in Personal Data or any customer information
• Bellarine Active Ageing OT does not engage with any overseas entities, with which personal or health information would be transferred, appointed or disclosed

• The transmission and exchange of information is carried out at your own risk. We cannot guarantee the security of any information that you transmit to us, we send to third parties or any information you receive from us. Although we take measures to safeguard against unauthorised disclosures of information, we cannot assure you that personal information that we collect will not be disclosed in a manner that is inconsistent with this Privacy Policy
How to access your personal information
• You may request details of personal information that we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth)
• If you would like a copy of the information which we hold about you or believe that any information we hold on you is inaccurate, out of date, incomplete, irrelevant or misleading, please email jen@baaot.com.au
• An electronic copy of the requested records will be provided, no hard copy will be provided, even when requested. Bellarine Active Ageing OT aims to be a 100% paperless service
• We reserve the right to refuse to provide you with information that we hold about you, in certain circumstances set out in the Privacy Act.
